New account holders are asked to sign up for service in person at City Hall. A driver's license or other photo identification are required, along with a non-refundable $25 service initiation fee.
If you are unable to make it to our office, e-mail a completed New Service Form along with a copy of your driver's license to email@example.com. Once received, a clerk will contact you to verify the information and collect a $25 non-refundable service fee, which is required before services can be started. The form and ID must be sent at least three business days before the start of services. Submission of form does not guarantee services until fee is paid and a clerk has given you a new account number for confirmation.
